Coming 1/1/2021 - For Families of Together with CCHP Members

Together with CCHP, an insurance program administered by Children's Community Health Plan, provides vision benefits for children 18 and younger utilizing our list of providers and locations. Professional Vision Services is pleased to offer enhanced P.V.S. Card benefits for the non-covered adult family members of Together with CCHP plan.  This discount card is accepted at all the same locations and providers as your children's benefit.  Please indicate that you are a Together with CCHP family when you enroll for your P.V.S. Card.
