Welcome to PVS (Professional Vision Services) - your one stop shop for all of your family eye care needs. Please note:  PVS is unable to schedule vision screenings for children 5 or under. Please follow up with your primary care physician in those instances.

WALK-IN AND SAME-DAY APPOINTMENTS   -  Are available most days at our HERSLOF MAYFAIR COLLECTION - WAUWATOSA, WI location and at our 

HERSLOF - HARTFORD, WI location.  

Call our CCHP PVS Customer Service Line    Simply call 1-800-501-0700 to reach one of our representatives who can assist with scheduling at any of our locations.  They are available to take your call Monday through Friday during the times listed to the right.
